Long Term placements care element fee per banding

The quoted prices are for the care element of the placement only. Please note that it is the expectation that all individuals have a meaningful weekday day time activity in place prior to the placement starting. If this is not the case, Shared Lives Carers (SLC) may receive an additional fee to provide day support on these days, if they are able. The level of day support required will be determined by the care plan and assessment of need. The day support fee will be negotiated on an individual basis. Service users will also need to pay SLCs a minimum housekeeping charge of £51.65 to cover the cost of food, heating, lighting etc. The individual will pay this fee directly to the carer. Where it applies, a housing benefit payment will also be paid to the SLC as a contribution towards rent.

The quoted prices are for the care element of the placement only. It is the expectation that individuals have a meaningful day time activity in place on weekdays at times of respite. If the service user is unable to access their day service whil they are on respite or they would like to use Shared Lives to have a holiday, the Shared Lives Carer (SLC) may receive an additional fee to support the individual in the day, if they are able. The level of day support required will be determined by the care plan and assessment of need and will be negotiated on an individual basis. All SLCs will also be paid a minimum housekeeping charge of £11.36 per 24 hour period to cover the cost of food, gas etc, which the service user will pay directly to the carer.
Long Term and respite placements
Care and support costs will be agreed during the referral and matching process in accordance with the purchase eligibility and the banding scheme below. Please do not go to panel until these costs have been discussed and agreed.
Bandings
Below is the banding scheme that will be applied to determine the Shared Lives Payment for both long term and respite placements.
Level One
The Shared Lives Carer will be required to undertake some of the following to meet the Adult’s assessed needs:
- Prompting regarding personal care and medication
- Emotional support and companionship
- Stimulation and encouragement
- Development or maintenance of daily living skills
- Introduction to community links and widening opportunities
- Maintain family contact and support network
- Intermittent supervision to ensure health, wellbeing and safety
Level Two
The Shared Lives Carer will be required to undertake the tasks at Level One, but more frequently and/or at a greater level e.g.
- Increased frequency of supervision
- Increased levels of emotional support
- More constant prompting
- More involvement in engaging the Adult in household and outside activities
Level Three
The Shared Lives Carer will undertake the tasks in levels one and two, but in addition will be supporting the Adult in at least three of the following areas:
- Personal care
- Management of single incontinence
- Assistance with eating and drinking
- Behaviour which is occasionally challenging
- Some disturbance during the night (occasionally)
Level Four
The Shared Lives Carer will be providing more than three of the tasks at level three and in addition up to three of the tasks below:
- Managing a significant level of risk as identified in a formal risk assessment
- Managing double incontinence
- Behaviour which is frequently challenging
- Frequent of sustained night-time disturbances
- More complex moving and handling tasks
Special Level
The provider will be meeting additional support needs over and above level four due to marked physical or challenging needs as identified in a specialist assessment.
Important Additonal Information
Transition costs
Most service users require some level of transition into a Shared Lives Placement. We try to be flexible as to what form this takes, as everyone is different, but please note that our carers are paid for this process. Charges will vary according to what the individual might need and so please contact us directly at the time of referral to discuss this further. Although there is no charge for the initial introductory visit, we will need confirmation of funding for all subsequent matching visits and the placement before we begin transition.
Long term SLC respite
Long term Shared Lives Carers are eligible for up to 6 weeks paid respite a year. The contract states that the exact amount will be allocated by Care Management. Please note that Care Managers are responsible for applying for this extra funding when they go to panel. During periods of respite the main SLC will continue to receive their housing benefit entitlement. The respite SLC will receive the full care element fee plus a housekeeping charge (minimum payment of £11.36 a night paid by the service user directly to the SLC).
Payments
SLCs will be paid in advance according to the terms of the Placement Agreement. In the event of an adult being absent from long term placements (for example following a hospital admission) the contract states that care and support costs will be paid at a rate of 50% for four weeks after which a Care Manager will review the arrangement.
