What referral documents do we need?
Referral to the Shared Lives Scheme must include the following supporting documentation:
- in the first instance, if you have not got the below paperwork to hand, please just send us through a pen picture telling us a bit about the service user and their needs and requirements so that we can start highlighting potential carers for them. This registers a formal interest in the scheme.
However, please note that, as per terms of our contract with SCC, a full referral must include the following documents:
- Shared Lives referral form
- A recent and detailed needs assessment For long term placements this assessment should ascertain whether the adult has the capacity to enter into a ‘licence to occupy’
- Confirmation of funding Please do not got to panel funding until the scheme has agreed the banding level you have suggested and we have discussed what funding is required for the placement as a whole (including any transition and respite costs). Please refer to the bandings and payments documents for more information about what these fees might be. Although there is no charge for the initial introductory visit, we will need confirmation of funding for all subsequent matching visits and the placement before we begin transition. Before the placement begins, we also need a signed service package. If service users have their own budget, we will need a written statement from them or yourselves to confirm what service they would like, that funding is in place and details on who holds their budget so we/the carer know who to invoice.
- Current Care Plan Our contract with SCC states that ‘the named assessor will write a care plan’ and that this will set out ‘how the SLS can assist the SU in meeting his or her needs and will describe the service or services that are to be provided.’
- Risk Assessment Our contract states that the referral will include ‘a current risk assessment completed by the Care Manager’. This assessment needs to assess any risks for the SU in a Shared Lives placement.
