How to make a referral

In suffolk, family carers may refer their adult to the Shared Lives Scheme by phoning Customer First on 0808 8004005.

Adults with a personal budget may apply direct to the scheme if they wish.

An initial referral will be completed for the adult.  When a suitable match is found, a social worker will work with your family and the Shared Lives Scheme to complete the necessary care and funding paperwork.  A placement agreement is drawn up and signed by all parties as to what care and support will be provided.
