How to make a referral

In Norfolk, families need to contact their care manager or call Norfolk County Council switchboard on 0844 8008014, if they do not have a care manager.

Adults with a personal budget may apply direct to the scheme if they wish.

An initial referral will be completed for the adult.  When a suitable match is found, a social worker will work with the family and the Shared Lives Scheme to complete the necessary care and funding paperwork.  A placement agreement is drawn up and signed by all parties as to what care and support will be provided.
