Respite care for 16-17 year olds
The Shared Lives Scheme in Norfolk has been extended to cover young people age 16 and 17. The service offers respite throughout the county and supports the young people and their families through the transition to adulthood, where established links may continue.
Jan Hooks, the Shared Lives Scheme Worker for Young People’s Respite, works 15 hours per week to assess carers and make placements after careful matching and introductions have been made.
For further information please contact Jan at the Diss office on 01379 644423 or 07775 464848, or by e-mail jhooks@guidepoststrust.org.uk. Further information about respite services for young people provided Norfolk County Council can be found at this website. http://www.time-for-me.org.uk
