Older Persons’ Floating Support

Guideposts trust offers a Floating Support Service to older persons’ in their own home.  As people grow older they may need extra support to continue to live independently and we provide this service to reflect these changing needs. If you are over 60 and receiving housing benefit the service is free, if you do not receive this then there is a possibility you may be charged.

We believe that all people have a right to an equality of opportunity.

 

What help we can offer?

Help sorting benefits and bill payments. (Are you struggling to budget effectively.)

Maintaining your tenancy. Is it at risk? Do you want to move?

Referrals to other agencies for cleaning and shopping.

Escorting to GP and other Health appointments.  (Prescription, medication collected)

Help and encourage you to arrange social activities

Check health and safety issues

Liaise with agencies for aids and adaptations

Culture specific health issues.

 

Coming out of hospital?

If you have recently been discharged from hospital, we can help you. For up to six weeks we can help with essential shopping and prescription collection whilst sourcing more permanent assistance.
